Baja wheel adapters keep coming loose

I bought a used truck that has these and the rear wheel came off and the front keeps tightening, I tightened the set screws, whats going on?

Take a file and score the axle where the set screw hits. Then use red loctite when you put it back together and wait 24 hours before you run it.

use some actone to clean any grease or dirt off.

Thats the problem then. and there was only one set screw in. anyone have about 10 of the little set screws I can buy? Along with a couple hub screws? Also, the original tool, the 4 way wrench that comes with the truck when new. I lost mine. I can pay paypal,
